I authorize Dr. Padmashree Balsarkar to assess my medical history and to provide healthcare services on 'as is' and 'as available' basis, including administration of drugs as deemed necessary. I am aware that healthcare services will be provided through telephonic or internet consultation with the Doctor and that there will be no physical examination. During the course of the treatment I will disclose sensitive personal information("SPI") which will include without limitation:

Physical, physiological and mental health condition, symptoms and history
Medical test results in connection with the aforesaid
Medical records and history

Doctor will not publish and disclose the SPI to any third person or body corporate without my express written consent, except when mandated by law. I may review the medical history and other related records provided by me to the Doctor and request the Doctor to correct or amend any inaccurate or deficient information. Doctor will not be responsible for the authenticity of the SPI provided by me to the Doctor. I agree that the Doctor liability will only be limited to the professional services rendered by it and that the Doctor does not make any guarantee, representations or endorsements or implied or express warranties with respect to the services provided by her.

About The Consultant

Dr. Padmashree R Balsarkar is a renowned gynaecologist obstetrician and infertility specialist practising since last 21 years in Thane.

She completed MBBS from the prestigious Grant Medical College and did her post graduation from Wadia Maternity Hospital and Lokmanya Tilak Muncipal Hospital.

She worked as a lecturer at Cama, Albless hospital (branch of Grant Medical College and JJ groups of hospital) and Bhabha Hospital.

She is a director of Saikrisha Maternity Hospital since last 15 years.

During her 21 years of practice she found that counselling her patients and making them understand their medical issues has helped them heal faster.
